Sick Notes (Fit Notes) and Self-Certification

Self-Certification

If you are ill for less than seven days, you do not need a Fit Note. You should use a Self-Certification (SC2) form.

You can get this form from your employer, from the HMRC website or you can download a copy below.

 

If you or your employer asks for a sick note during the first 7 days, a charge will apply.

Sick Notes from the Practice (More Than 7 Days)

If you’ve been off sick for more than seven days, your employer may ask for a Fit Note (Statement of Fitness for Work). You can request this from:

  • A GP
  • Our Advanced Nurse Practitioner, Sooz

Please note:

  • Sick or Fit Notes can be backdated, so please discuss your specific dates when speaking with us.
  • Urgent or emergency appointments should not be used to request Sick or Fit Notes.

If you believe that you will likely need a Sick or Fit Note before you reach the 7th day of absence, please contact us in advance. Our Reception Team will aim to book a routine appointment as close to day 7 as possible to ensure there’s no delay in providing your note.  

Hospital Admissions

If your time off work was due to a hospital admission:

  • The hospital should issue a Sick Note to cover the period you were admitted.
  • Once discharged, the practice can issue a Fit Note based on your hospital discharge letter, please book a routine appointment via reception.

Requesting an Extension

If your illness continues beyond the period covered by your initial sick note, you will need to request an extension. Please request an extension online by following the link below.

If you need a further consultation with a GP, the practice will contact you directly.
