Register as a new patient

Use this page if you would like to apply to register as a patient with Eastfield Medical Practice.

You can usually register permanently with a GP practice if you are living in the area for more than 3 months. If you are staying locally for less than 3 months, you may still be able to access care as a temporary resident.

Check the practice boundary

Please check your address before completing the registration form.

If your address is outside our practice area, your application cannot be accepted and you should register with a GP practice that covers your home address.

Complete the registration form

Once you have checked that you live within our practice area, please complete the NHS Scotland GP registration form.

Please complete the form as fully as possible, and let us know if you are unsure.

Return the form to the practice

You can return your completed form to the practice by:

  • bringing it to reception
  • emailing it to us

Please ensure you have signed the form on the back.

What happens next

Once we receive your registration form, we will check that the form is complete and that your address is within our practice area.

We will contact you if we need more information.

Please allow time for your registration to be processed before requesting routine appointments, prescriptions or administrative services.
